Program Description: Patients with wounds can present a challenge for home health agencies who may not have the frame work or expertise to manage wound patients optimally. This presentation offers “Best Practice” solutions and highlights major issues and problems faced by home health and hospice agencies when delivering wound care in the home. Topics include:

  • legal responsibility
  • timely care delivery
  • misdiagnosis of wounds
  • OASIS documentation
  • wound classification
  • dressings (cost, purchasing, formulary development)
  • staff education
  • development of wound management protocol

Program Objectives: Upon completion of this program, the viewer will be able to:

  • Identify quality issues in managing clients with wounds
  • Describe Outcome based quality monitoring (OBQM) and managing clients with wounds
  • Identify wounds requiring Home Health Care and common wound care issues faced by home health agencies
  • Describe assessing wounds, appropriate documentation, types of wounds, types of wound dressings, and best practices in managing home clients with wounds.

About the Presenter:
Alex Kahn
is CEO of Pro Healthcare Home Visiting Wound / Ostomy / Foot Care Care Services. He has 17 years of Home Health experience and 21 years of Nursing experience. Alex is a Wound Care Expert and Former U.S. Army Nurse, trained at Fort Sam Houston, San Antonio, Texas, Brooks Army Medical Center Burn Unit. He formulated and Directed a Hospital-wide Wound Care program at Dallas VA Medical center and is a former member of the Wound Ostomy Continence Certification Board. He is also a Clinical Preceptor and faculty member at Emory University Wound Care program, in Atlanta, Georgia.
