Program Description: The home health industry is confronting a period of great change that will challenge all community service providers as healthcare reform reinvents how care is produced, managed, and delivered. Homecare veterans who experienced the shift from fee-for-service to the prospective payment system (PPS) will recognize familiar echoes of the past as our industry proceeds to remake clinical programs to serve the changing needs of our care community.

Led by the Affordable Care Act (ACA) healthcare reform efforts, designed to create new efficiencies and improved care while reducing unnecessary costs, home health faces a unique opportunity. The ACA emphasizes post-acute care management as the key to assuring acute care value. Home health represents the solution to managing readmissions and controlling inpatient care costs. But the homecare industry of today must implement value-based, best practice protocols and care management models from outside of the home heath world if they hope to answer the needs of the ACA episodic model.

This progressive presentation outlines the areas home health providers should address as they rewire care processes to achieve the goals of tomorrow. Learn how some home health providers are addressing these challenges, and producing the care programs of tomorrow today. Donít miss the chance to rewire your programs to achieve the requirements of the ACA, while simultaneously achieving more efficient episodes focused on the real drivers of clinical outcomes. Presentation Date: 08.12.2015

Program Objectives: Upon completion of this program, the viewer will be able to:
