Program Description: Jennifer Markley, Director of the Quality Improvement Organization's statewide initiative to reduce readmissions presents an update on project participation, interventions, year one outcomes and the state's progress towards reducing readmissions by 20%.

Program Objectives: Upon completion of this program, the viewer will be able to:

  • Discuss resources and support available to professionals working to reduce readmission rates, including the Texas Readmissions Learning and Action Network.
  • Analyze improvements in readmissions rates at the end of year 1 of the project to improve readmission outcomes by 20%.
  • Evaluate best-practice interventions to adopt into care delivery.

About the Presenter: Jennifer Markley, RN, BSN is the Senior Director for Quality Improvement at TMF Health Quality Institute, where she directs TMF’s 2011-2014 Texas readmissions initiative funded through CMS. She also currently oversees a contract working with American Institute of Research (AIR) to support Learning and Diffusion for CMMI projects. In 2008-2011, Ms. Markley led a Care Transitions pilot for Texas, where she oversaw a team that worked with stakeholders to decrease 30-day readmissions to hospitals. Texas was one of 14 pilot sites for this work, and Texas’ improvements in this areas accounted for 30% of the overall improvement in the pilot project. Ms. Markley is a registered nurse with 29 years of health care management experience and 37 years of nursing experience.
