About the Presenters: Rhonda Will, RN, BS, HCS-D, COS-C heads Fazzi's OASIS auditing team and is one of home care's most well known national trainer and speaker. Cindy Krafft , MS, PT, COS-C is an Assistant Director of Operational Consulting, Rehab Services at Fazzi Associates, Chair of the NAHC Therapy Advisory Committee and the current Vice President of the Home Health Section of the American Physical Therapy Association.

Interdisciplinary Orientation: Key Concepts in Home Health Care

This program is Part One of the the Fazzi Associates Interdisciplinary Orientation to Home Health Care Series. In this program Rhonda Will and Cindy Krafft review fundamental concepts of home care. They discuss the three primary roles of clinicians in the provision of home care services and explain the "basics" of Medicare's payment structure. Throughout this program they emphasize home care's unique attributes.


Interdisciplinary Orientation: Medicare Conditions of Participation

This program is Part Two of the Fazzi Associates Interdisciplinary Orientation to Home Health Care Series. In this two-part program Rhonda Will and Cindy Krafft provide a detailed explanation of Medicare's Conditions of Participation for home care providers. Cindy and Rhonda review, in detail, Medicare beneficiary eligibility and coverage criteria, focusing particular attention on the "homebound" definition.


Interdisciplinary Orientation: Identifying & Delivering Skilled Care in a Home Setting

This program is Part Three of the Fazzi Associates Interdisciplinary Orientation to Home Health Care Series. In this program, Rhonda Will and Cindy Krafft discuss the delivery of skilled services to patients in a home-based environment. They stress the uniqueness of home care and highlights adaptations that need to be made when delivering services in home as opposed to facility-based settings.


Interdisciplinary Orientation: Documentation, A Critical Element of Quality Care

This program is Part Four of the Fazzi Associates Interdisciplinary Orientation to Home Health Care Series. Preparing complete and accurate documentation is a critical role of home care clinicians. In this program, Cindy Krafft discusses the need for consistency in medical records and examines documentation strategies to support "medical necessity."


Interdisciplinary Orientation: Comprehensive Assessment, OASIS & Outcomes

This program is Part Five of the Fazzi Associates Interdisciplinary Orientation to Home Health Care Series. In this course module, Cindy Krafft and Rhonda Will discuss the comprehensive assessment process and its relationship to achieving positive patient outcomes. Throughout this discussion they emphasize the importance of OASIS accuracy and explore the role of outcome measures in determining quality of care.


Interdisciplinary Orientation: Care Management - Successful Strategies for a Team Approach

This is Part Six of Fazzi Associates Interdisciplinary Orientation to Home Health Care Series. In this final session, Rhonda Will and Cindy Krafft examine strategies for successfully implementing a team-oriented, patient-focussed care approach and stress the significance of collaboration among clinicians.
