ON JANUARY 1, 2017 a new version of the OASIS data set and updated guidance was implemented - OASIS-C2. This latest revision of OASIS includes several changes to bring home health assessment data collection into alignment with other post-acute provider settings:

  • Formatting changes the overall look of the OASIS data items in many areas.
  • Three new standardized items:
    • M1028 (Active Diagnoses - Co-Morbidities and Co-existing Conditions)
    • M1060 (Height and Weight)
    • GG0170c (Functional Abilities and Goals)
  • Changes in content and numbering for several integumentary items:
    • M1311 (Current Number of Unhealed Pressure Ulcers at each Stage)
    • M1313 (Worsening in Pressure Ulcer Status since SOC/ROC)
  • ...and medication items:
    • M2001 (Drug Regimen Review)
    • M2003 (Medication Follow-up)
    • M2005 (Medication Intervention).
  • Pressure ulcer items have new guidance from the NPUAP and numbering of pressure ulcer stages change from Roman to Arabic numerals.

Presenter and Subject-Matter Expert: Lisa Selman-Holman, JD, BSN, RN, HCS-D, HCS-H, COS-C, HCS-O

About the Presenter: Lisa Selman-Holman, JD, BSN, RN, HCS-D, HCS-H, COS-C, HCS-O is a veteran of home care with over 27 years spent in home health and hospice, both as an RN and as an attorney practicing exclusively in home care. She is the owner of Selman-Holman & Associates, LLC, a full-service home care and hospice consulting firm, CoDR (Coding Done Right), an outsourcing company for home care and hospice coding and OASIS review, and CodePro University, developing and providing ICD-10-CM code training for home health and hospice coders and OASIS training for home health. She is the Chair of the Board of Medical Specialty Coding and Compliance specialty board on OASIS and home care/hospice coding. She has provided education to home health and hospice professionals since 1994.

Lisa is certified as a Homecare Coding Specialist-Diagnosis (HCS-D), Homecare Coding Specialist-Hospice (HCS-H), Homecare Coding Specialist-OASIS (HCS-O) and is certified in OASIS competency, COS-C. She is the technical editor to Decision Health's Complete Home Health ICD-10-CM Diagnosis Coding Manual and an author of the Decision Health OASIS-C2 study manual. She is the leader of the SME team for OASIS Optimize by Ability. She is a frequent contributor to other home care periodicals. Lis is a BMSC-approved instructor for the HCS-D exam and an AHIMA-approved ICD-10-CM Trainer. She has participated on the OASIS Certificate and Competency Board on the Exam Review Task Force. She is an active member of the Texas Association for Home Care & Hospice and other state associations and frequently serves on regulatory task forces. She has also participated with the Texas Medical Foundation's Quality Improvement Organization State Home Health Coalition for OBQI.

  • understand the purpose and uses of the OASIS patient assessment data
  • apply the training and CMS guidance to correctly and accurately answer OASIS data items
  • maintain compliance with applicable federal regulations governing OASIS

Requirements for Successful Completion: To successfully complete this activity, the participant must complete all components of the training format (online training presentations, integrated quizzes, program post-tests), achieve an 80% or higher on the 80-question series-cumulative post-test and complete the series evaluation.

Nurse CE Accreditation Statement:
Selman-Holman & Associates, LLC is an approved provider of continuing nursing education by the Texas Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

Requirements for Successful Completion
Desired Outcome: The Home Health RN will actively participate in the learning activity and indicate an intenet to change or enhance their practice by using the official guidance to respond to OASIS items and to direct nursing assessments and documentation of clinical information to support skilled care, enhance RN communication with physicians and establish plans of care resulting in optimal patient outcomes.
Learning Outcome Statement: Participants will view all program segments in their entirety and will indicate an intent to change practice on their post-activity evaluation tool. To receive credit for this continuing nursing education activity, the participant must:

  • view each program in its entirety
  • successfully complete program quizzes and program post-tests
  • complete the series evaluation
  • successfully complete the series cumulative post-test

Once successful completion is verified, a “Certificate of Successful Completion” will be awarded for 19.0 contact hours.
